COMMUNITY NEEDS ASSESSMENT

Columbia County Public Health is embarking on a Community Needs Assessment and strategic planning process to develop a vision and strategic plan for the future of our local community. This process will serve as an opportunity for members of the community to provide their input to developing this vision.  The Board has appointed Jasmine Helm as a member of an advisory committee tasked with leading the outreach in our community.  We believe our community members will have vital input to this process so we would like to solicit your help. Please take this survey and pass it along to friends and family who live in, work in and care about our community. Your input is vital to the decision making process regarding programs that we will be able to bring to our community.
